This Privacy Policy sets out our commitment to protecting the privacy of personal information provided to us, or otherwise collected by us when providing our Services to you. Personal information: The types of personal information we may collect about you include:

  • your credit card or payment details (through our third party payment processor);

  • drivers licence number;
  • Medicare details;
  • ImmiCard; or
  • Australian Passport.

Unless otherwise permitted by law, we will not collect sensitive information about you without first obtaining your consent.

HOW WE COLLECT PERSONAL INFORMATION

We collect personal information in a variety of ways, including:

  • Directly: We collect personal information which you directly provide to us, including when you register for an account, through the ‘contact us’ form on our website or when you request our assistance via email, our online chat or over the telephone.
  • Indirectly: We may collect personal information which you indirectly provide to us while interacting with us, such as when you use our website, in emails, over the telephone and in your online enquiries.

The sole purpose of passively collecting your information is to improve your experience when using our website and web services.

  • to provide our Services to you, including consulting services in relation to the NDIS, Home Care and Aged Care registration and accreditation support and advice;
  • to enable you to access and use our associated applications;
  • to contact and communicate with you about our Services;
  • for internal record keeping, administrative, invoicing and billing purposes;
  • for analytics, market research and business development, including to operate and improve our Services, associated applications;
  • for advertising and marketing, including to send you promotional information about our products and services and information that we consider may be of interest to you;
  • to comply with our legal obligations and resolve any disputes that we may have;
  • if you have applied for employment with us; to consider your employment application; and
  • if otherwise required or authorised by law.

Sensitive information: We only collect, hold, use and disclose sensitive information for the following purposes:

  • any purposes you consent to;
  • the primary purpose for which it is collected, when you have requested us to assist with NDIS or Home Care (MyAgedCare) registration and online portal set up;
  • secondary purposes that are directly related to the primary purpose for which it was collected, including disclosure to the below listed third parties as reasonably necessary to provide our Services to you;
  • to contact emergency services, or to speak with your family, partner or support person where we reasonably believe there is a serious risk to the life, health or safety of you or another person and it is impracticable for us to obtain your consent; and
  • if otherwise required or authorised by law.

DISCLOSURE OF PERSONAL INFORMATION TO THIRD PARTIES

Personal Information: We will only disclose personal information (excluding sensitive information) to third parties where it is necessary as part of our business, where we have your consent, or where permitted by law. This means that we may disclose personal information (excluding sensitive information) to:

  • third party service providers for the purpose of enabling them to provide their services, to us, including (without limitation) IT service providers, data storage, web-hosting and server providers, debt collectors, couriers, maintenance or problem-solving providers, marketing or advertising providers, professional advisors and payment systems operators;
  • other community members as part of our community platform;
  • our employees, contractors and/or related entities;
  • professional advisors, bankers, auditors, our insurers and insurance brokers;
  • payment systems operators or processors;

OVERSEAS DISCLOSURE

We store your personal information in Australia. Where we disclose your personal information to third parties, those third parties may store, transfer or access personal information outside of Australia, including but not limited to, Brunei, the Philippines and Pakistan. We will only disclose your personal information overseas in accordance with the Australian Privacy Principles.

YOUR RIGHTS AND CONTROLLING YOUR PERSONAL INFORMATION

Your choice: Please read this Privacy Policy carefully. If you provide personal information to us, you understand we will collect, hold, use and disclose your personal information in accordance with this Privacy Policy. You do not have to provide personal information to us, however, if you do not, it may affect our ability to provide our Services to you and your use of our Services.

If you are a third party providing personal information about somebody else, you represent and warrant that you have such person’s consent to provide the personal information to us.

Access: You may request access to the personal information that we hold about you. An administrative fee may be payable for the provision of such information. We are committed to ensuring that the personal information we collect is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures, to safeguard and secure personal information and protect it from misuse, interference, loss and unauthorised access, modification and disclosure.

While we are committed to security, we cannot guarantee the security of any information that is transmitted to or by us over the Internet. The transmission and exchange of information is carried out at your own risk.

We may use cookies on our online Services from time to time. Cookies are text files placed in your computer’s browser to store your preferences. This policy confirms Provider Group Pty Ltd t/as Provider plus commitment to protecting the privacy of your personal information, in accordance with the Australian Privacy Principles (APPs), and as stipulated under the Privacy Act, 1988.

1.1 Collection of your personal information

We collect information directly from you, to provide you with services that meet your business needs. Personal information we may collect and hold includes:

  • Your name, address and email address
  • Telephone number(s)
  • Business details including contact details and website
  • Information concerning your business and its needs including financial information, business planning and cost modelling.

Where you have requested us to assist with NDIS or HomeCare (MyAgedCare) registration and online portal set up, we will need you to provide us with identifying documents, such as

  • Driver’s licence number,
  • Medicare details,
  • ImmiCard,
  • Australian Passport and, if required, evidence of change of name.

All sensitive information (Usernames, Medicare, Passport details etc) will be collected either:

  • by scanning and emailing directly as a PDF to Provider Group Pty Ltd t/as Provider plus;
  • or via FormStack. This application protects users’ data from their browser to their inbox through multiple levels of form security.

1.2 Why we collect personal information

We collect and hold personal information to provide you with a business service in relation to community sector and NDIS or HomeCare consulting, direct marketing and communications and other services you request of us.

If you receive any material from us that you did not request or do not wish to receive in future, please contact us as set out at 1.7 below and we will remove your details from future marketing.

1.3 Disclosure of your personal information

We will not transfer, sell, assign or rent your personal information. We will not disclose any of your information without your consent, unless we are authorised or required to do so by law.

Information provided to us to establish your Provider Digital Access (PRODA) account will be provided to the Department of Human Services so you can use the account. The department’s privacy statement, which advises that your personal information will not be shared outside the department, can be viewed here.

Once your account is activated, we will destroy our copies of your identity information that we used to create the account.

We securely store the personal information you provide using this service. Your personal information is not shared outside of the department.

You have a right to have your personal information kept private. We are bound by the Privacy Act 1988 and the department’s confidentiality and secrecy provisions.

1.4 How we hold personal information securely

Provider Group Pty Ltd t/as Provider plus holds and stores your personal information within a controlled third-party application that has been contracted by us.

The application has procedural safeguards in place that comply with federal regulations to protect personal and business information about you.

If you believe the information we hold to be outdated, incomplete, or inaccurate you may seek correction. We will give you written notice if we are not able to make a correction, setting out the reasons and the mechanisms by which you can make a complaint.
